Sup. Ct. R. App. P. 4(a).\u00a0 The notice must be filed with the trial court\u2019s clerk\u2019s office, not the Supreme Court\u2019s clerk\u2019s office.\u00a0 <i>Id<\/i>.\u00a0 The form for filing a notice of appeal may be found in the trial court\u2019s clerk\u2019s office.\u00a0 If there is information that does not fit within the allocated space on the form, it is common practice to attach an addendum to the notice form.\u00a0 <i>See<\/i> <a href=\"http:\/\/www.courts.ri.gov\/Courts\/SupremeCourt\/Opinions\/13-63.pdf\" target=\"_blank\" rel=\"noopener\">Miller v. Metropolitan Property and Casualty Ins.
